Student Profile

This profile is in two sections. Section one contains information that may be answered by just supplying the appropriate answer--emailed to me if possible. Section two should be a short biography, concentrating on any or all of the questions; the questions should be answered in paragraph form followed by the agreement, copied and signed by each student. You need not answer section two's questions in order--in fact, you may choose to answer your own questions; I'd prefer you be creative.
